Your health information
This notice describes how medical information may be used and disclosed and how you can access that information. This Notice applies to the independent professional medical entity, Umsha Health LLC, and the licensed clinicians providing care through the ME & YOU platform, which together operate as the covered entity for this Notice. ME & YOU Health LLC acts as a business associate providing technology and administrative services under a written business associate agreement. Effective date: September 2026. We are required by law to maintain the privacy of protected health information, to give you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
Uses and disclosures
We may use and disclose your health information without your authorization for:
- treatment — sharing your intake, labs, and history with the clinician reviewing your care, with a pharmacy to dispense a prescription, and with other providers involved in your care
- payment — obtaining payment for services, verifying coverage, and billing
- health care operations — quality review, clinician licensure and credentialing, training, audits, and business management
- as required by law — including court orders, subpoenas, and mandatory reporting
- public health and safety — reporting adverse drug events to the FDA, communicable disease reporting, suspected abuse or neglect, and to prevent a serious and imminent threat
- health oversight, law enforcement, coroners, organ procurement, workers’ compensation, and research where permitted by HIPAA
- persons involved in your care where you agree or where it is in your best interest and you are unable to agree
Any other use or disclosure — including most uses of psychotherapy notes, marketing, and any sale of health information — requires your written authorization, which you may revoke in writing at any time except where we have already acted on it.
Your rights
You have the right to:
- inspect and receive a copy of your health information, including an electronic copy, usually within 30 days and for a reasonable cost-based fee
- request a correction of information you believe is inaccurate or incomplete (we may deny and you may file a statement of disagreement)
- receive an accounting of certain disclosures made in the 6 years before your request
- request a restriction on uses and disclosures (we are not required to agree, except that we must agree to withhold information from a health plan for care you paid for in full yourself)
- request confidential communications by a particular method or at an alternative address
- receive a paper copy of this Notice on request
- be notified of a breach of unsecured health information
- choose someone to act for you with a valid power of attorney or as a personal representative
Our responsibilities
We are required by law to protect the privacy and security of your health information, to notify you promptly if a breach compromises the privacy or security of that information, and to abide by the terms of this Notice. We will not use or share your information other than as described here unless you give written permission. We may change this Notice, and the change will apply to all information we hold; the current Notice is always posted on this page with its effective date. You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201, 1-877-696-6775, or at hhs.gov/ocr/privacy/hipaa/complaints. We will never retaliate against you for filing a complaint.
